Why an NCR has to trace back to a requirement

A non-conformance report exists to prove a decision, not just record a complaint: cite the exact specification clause, drawing revision, or contract requirement the work fails to meet, describe what was actually found, and attach the evidence — a photo, a test result, a measurement — that a reviewer can check independently.

Disposition (use as is, repair, rework, or reject and replace) needs a named approver and a justification tied back to the same requirement, because it is the record an auditor or client will ask to see again later. Root cause and corrective action address this occurrence; preventive action is the separate, harder question of what stops it recurring elsewhere on the project.

What is a non-conformance report used for?

It is raised when completed work, delivered materials, or a process do not meet a specified requirement — a quality, safety, or contract requirement. It records the requirement that was missed, the disposition decided on the non-conforming item, and the corrective and preventive action, and stays open until verification confirms the fix.

Who raises an NCR?

Typically the QA/QC inspector, site or quality engineer, or the client's consultant, usually during an inspection or a test. On some contracts a contractor's own quality team self-reports a non-conformance rather than waiting to be found.

What is the difference between disposition and corrective action?

Disposition decides what happens to this specific non-conforming item right now — use as is, repair, rework, or reject and replace. Corrective action addresses why it happened and what stops the same failure recurring. Both belong on the same record, but they answer different questions and are usually approved separately.
