Its purpose is to make sure hazards are identified, precautions and isolations are in place and verified, and the area is confirmed safe again before the permit is closed.

Who should sign a permit to work?
Typically an authorised person issues the permit and the performing party, or their supervisor, accepts it, confirming they understand and will comply with its conditions. High-risk permits also record a close-out by whoever confirms the work area has been left safe. This form captures names, roles, and times as text — it is not an electronic signature.
